Normal Labs but Still Feel Awful? What to Do Next

"My labs are normal but I still feel awful." If you have typed some version of that into a search bar, this article is for you, and the first thing to say is the most important: you are not imagining it, exaggerating it, or simply stressed. This is one of the most common experiences in modern primary care. A standard blood panel is a starting point, not a full picture of your health. Here is what it misses, which tests to ask for by name, and how to advocate for a proper investigation without turning your doctor into an adversary.

Why normal labs do not always mean you are well

A routine blood panel is broad but shallow. It is designed to catch common, obvious problems cheaply, not to explain every symptom. Several real gaps sit inside a "normal" result.

  • Ferritin is often not included. A standard full blood count can look normal while your iron stores are running empty. You can be iron-depleted and genuinely symptomatic before your haemoglobin ever drops.
  • A normal TSH does not rule out thyroid trouble. Thyroid-stimulating hormone within the laboratory range can still sit at a level that leaves some people symptomatic.
  • Vitamin D and B12 are not standard everywhere. In many countries they are only tested if you ask, yet both cause fatigue and low mood when low.
  • Inflammatory markers can be normal in low-grade disease. CRP and ESR can sit in range during early or mild autoimmune conditions.
  • Hormones are almost never in a standard panel. A routine workup rarely looks at sex hormones or cortisol, despite how symptomatic these are when dysregulated.

A normal panel rules some things out. It does not rule everything in.

There is also the question of what "normal" even means. A laboratory reference range is built from the spread of results in a large population, and being inside it means only that you are not an outlier, not that the value is optimal for you. Two people can both be "in range" while one feels well and the other does not. Ranges also vary between laboratories, and a result near the very top or bottom of a range can be worth a second look if your symptoms fit. Normal is a statistical statement, not a verdict on how you feel.

Conditions that standard labs frequently miss

This is the heart of the matter. Several real, recognised conditions do not show up on a basic blood test, which is exactly why people with them are so often told everything looks fine.

Subclinical hypothyroidism

Here TSH sits in the upper part of the "normal" range while free thyroid hormone is low-normal, and symptoms persist: fatigue, weight gain, cold intolerance, low mood. There is genuine clinical debate, reflected in NICE guidance, about the point at which this should be treated, which means a single in-range TSH is not the end of the conversation if you remain unwell.

Ferritin deficiency without anaemia

Ferritin below about 30 can cause fatigue, hair loss, restless legs and brain fog well before haemoglobin falls and anaemia is diagnosed. Because the full blood count looks normal, this is one of the most commonly missed causes of unexplained tiredness, particularly in menstruating women, in whom monthly blood loss steadily depletes stores. It is also easily checked and easily treated, usually with a course of iron and attention to the underlying cause, which makes it one of the most rewarding things to look for.

SIBO

Small intestinal bacterial overgrowth causes bloating, wind, irregular bowels and fatigue, and it appears on no standard blood panel. Diagnosis needs a breath test, so it is invisible unless someone thinks to look for it.

Long COVID and post-viral syndromes

Persistent fatigue, breathlessness, brain fog and post-exertional malaise after an infection are real and increasingly recognised, yet routine bloods are typically normal. Post-exertional malaise, where symptoms worsen sharply a day or two after even mild activity, is a particularly distinctive feature and one that standard tests cannot capture. These remain clinical diagnoses based on symptom patterns over time rather than a single test, which is why a clear record of how your symptoms respond to activity is so useful to the clinician assessing you.

Hypermobile Ehlers-Danlos syndrome and hypermobility

This is a connective tissue disorder that causes widespread joint pain, fatigue, frequent injuries and often digestive and autonomic symptoms. There is no routine blood test for the common hypermobile type; it is a clinical diagnosis, which is why it is so often missed for years.

POTS

Postural orthostatic tachycardia syndrome causes a large rise in heart rate on standing, with dizziness, palpitations, fatigue and brain fog. Standard labs are normal. Dysautonomia International notes it is diagnosed with an active stand test or a tilt-table test, not bloodwork.

Mast cell activation syndrome

Mast cells are part of the immune system, and in this condition they release their chemical signals inappropriately, causing flushing, itching, hives, digestive upset, palpitations and reactions to foods, smells or temperature changes. It frequently overlaps with hypermobility and POTS, and it does not show up on a routine panel. It is under-recognised and often mistaken for unrelated allergies or anxiety.

Reactive hypoglycaemia

Standard fasting glucose is measured when you have not eaten, so it can completely miss reactive hypoglycaemia, where blood sugar drops sharply a few hours after a meal. The result is shakiness, sweating, irritability, brain fog and fatigue that a fasting test taken first thing in the morning will never capture. It takes a symptom diary tied to meals, and sometimes a longer glucose study, to see it.

Perimenopause

In women under 45, perimenopause is frequently missed because FSH is unreliable during this transition: a "normal" result does not rule it out, since hormones fluctuate day to day. Symptoms often arrive years before periods change, and they range far beyond hot flushes to include anxiety, joint pain, palpitations, poor sleep and brain fog, which is exactly why they get attributed to everything except hormones. How to advocate for further investigation

Being unwell with normal results is frustrating, but you can steer the process. Here is how.

Ask for specific tests by name. Rather than "can we do more bloods", request the ones that fill the gaps above: ferritin (not just haemoglobin), vitamin D as 25-OH-D, vitamin B12, a morning cortisol, a full thyroid panel including free T3 and thyroid antibodies rather than TSH alone, and an ANA screen if you have autoimmune-type symptoms. Frame it as one picture, not a list. Ask "what could cause all of these symptoms together?" rather than presenting each symptom separately. Connected symptoms point towards a diagnosis; scattered ones get dismissed one by one.

Bring evidence. A symptom diary with dates, severity scores and patterns is far more persuasive than "I've just felt off". It also helps your doctor spot a pattern a single ten-minute appointment cannot.

Ask about a referral. If your GP has exhausted their standard workup and you remain unwell, it is reasonable to ask for a specialist opinion. A second opinion is not an insult, it is a normal part of care. Which specialist depends on the pattern: an endocrinologist for suspected hormonal or thyroid problems, a rheumatologist for widespread joint pain and autoimmune-type symptoms, a gastroenterologist for persistent gut symptoms, or a cardiologist for palpitations and fainting.

Book a longer appointment if you can. A standard slot is rarely enough to work through a complex, multi-system picture. Asking for a double appointment, and leading with your single most troubling symptom, gives the conversation room to breathe and makes it far more likely that the connections between your symptoms are actually explored.

When the answer might involve mental health

This section needs care in both directions. Depression, anxiety and functional disorders are real conditions that cause real physical symptoms, fatigue, pain, digestive upset, palpitations, through genuine physiological pathways. Dismissing physical symptoms as "just mental health" is inappropriate. So is refusing to consider mental health as a contributor when it may be part of the picture.

Conditions such as somatic symptom disorder and functional neurological disorder are recognised and treatable, and naming them is not a way of saying the symptoms are not real. The pain, the fatigue and the dizziness are genuinely experienced; the nervous system is producing real signals, even where a structural cause is not found. Chronic stress also has measurable physical effects, from disrupted sleep and raised cortisol to gut symptoms and muscle tension, which can themselves generate the very symptoms that prompt the search for a diagnosis. The most useful stance is that mental and physical health are not mutually exclusive. Investigating both at once is good medicine, not a sign that your doctor disbelieves you, and treatments that help the nervous system settle can ease physical symptoms even when every scan is clear.

The single appointment is the enemy of these diagnoses, because so many of them only become visible as a pattern over weeks or months. This is where keeping your own longitudinal record pays off. The bottom line

Normal blood tests are reassuring, but they are not the same as being well. Standard panels miss iron depletion without anaemia, subclinical thyroid problems, post-viral syndromes, dysautonomia, hypermobility and perimenopause, among others. If you remain unwell despite normal results, you are not out of options: you can ask for specific tests, present your symptoms as one connected picture, track the pattern over time, and request a referral. You know your body better than anyone. Being believed starts with being organised, and you deserve to be taken seriously on both counts.
